1. His pre-season workout with Miami must have been terrible, and word got around. He couldn't even get on a pre-season roster when so many terrible QBs with 3rd string ceilings and undraftable free agents got a look. The only time I saw his name mentioned all year was about his unimpressive workout with the Dolphins.
2. He did something with the Ravens and/or 49ers to earn a reputation as a malcontent, and word got around. I remember when Harbaugh was announced as new 49ers coach, some speculated that the other Harbaugh might put in a good word. Not a chance. The Ravens cut him for a reason, and before they cut him he was available at a very low cost and had no offers.
3. ( Related to #2): I think NFL personnel men see Troy as a 2nd/3rd string talent. He probably sees himself as a starter and is passionate about that. Sometimes that passion is misconstrued, and he's not going to be seen as fitting in as a backup. Someone like Matt Leinart, who appears to have no motivation or interest in being a starting NFL QB, is seen as a safer pick as a backup, sadly. For a 3rd stringer- some teams don't even carry a 3rd QB. For others, why sign a guy who is going to cost the 5th year veteran salary when you can let undrafted guys or late pick rookies who only cost the rookie minimum ( or you can stash on the practice squad for 90 grand a year) battle it out, for a position that most likely won't see the field?
4. His UFL performance/notice. It seems no one cares about the UFL, except they kinda do. He finally got to play in the last UFL game, and did OK. No one even mentioned it here. His team lost in overtime. He completed barely over 50% of his passes, 2 TDs, 1 Int. Nothing too impressive, but got game action finally. if he had signed with the UFL early and beat out Crouch and Masoli, maybe a "full" season there would have helped him out. I've seen many UFL QBs get workouts with NFL teams this year, even guys who were there in 2010. Profootballtalk makes a post every Weds listing all the free agents who get tryouts every Tues. So many terrible washout QBs and UFL guys get workouts. Mcleod Bethel-Thompson has tried out with at least 2 NFL teams since the UFL season ended. Who? He's the QB who beat Troy in the last UFL game. In fact both Bethel-Thompson and Masoli were with the 49ers, who had 0 interest in having Troy back, in the preseason.
5. Some teams WANT to lose. The players are trying to win, but the front offices are purposely not giving them the talent to for draft/cap reasons.
I think overall Troy is just seen as a very marginal NFL talent who got 4 years in the league and got a couple chances and hasn't shown much worth taking a chance on, even though we know he was THIS close to starting over Flacco and was no worse than anyone the 49ers started last season. Personally, I would take him over 12 of the guys who started this week, and certainly most of the backups.
Troy Smith arrested in downtown Cleveland

CLEVELAND, OH (WOIO) -
Former NFL'er, Troy Smith, was arrested in downtown Cleveland Friday evening.
Cleveland Police tell 19 Action News at approximately 9:30 p.m., Troy Smith was stopped at W. 9th and Main Avenue for a traffic violation. After checking Smith, he was found to have an outstanding warrant for Driving Under Suspension and place under arrest.
Smith played college football at Ohio State where he won the Heisman Trophy in 2006. He was then drafted by the Baltimore Ravens and later played for the San Francisco 49'ers.
