Some great ones:

-Deep Purple, "Hush" (Joe South) and "Kentucky Woman" (Neil Diamond)

-Yes, "Every Little Thing" (Beatles) and "America" (Simon & Garfunkel)

-Stevie Wonder, "We Can Work it Out" (Beatles)

-Nirvana, "The Man Who Sold the World" (Bowie)

-And I always liked GnR's version of "Live and Let Die" (Wings)

Oh, and one more deserving of mention is Mountain Jam by the Allman Brothers Band, granted their version has no lyrics. If you ever heard the original, 'There is a Mountain' by Donovan, you'll laugh your ass off when comparing it to the behemoth jam the Allman's turned it into.

Colonel Les Claypool's Fearless Flying Frog Brigade's album Live Frogs, Set 2 which is a start-to-finish live cover of Pink Floyd's entire Animals LP.

Worst:

The Duran Duran Tribute Album, which features Goldfinger's version of Rio, Reel Big Fish having fun with Hungry Like the Wolf, then 13 other forgettable (and mostly awful) punk/ska bands doing covers of 13 forgettable Duran Duran "hits." Most of these bands are so bad The Warped Tour won't have them back!

If you listen to the Grateful Dead's 2/14/68 Carousel Ballroom version of "Alligator" you can hear Garcia playing with this theme as well (Actually, you can listen to Anthem of The Sun's version which is taken in large part from the aformentioned show). It is said Duane was jamin on Alligator when he thought of Mountain Jam, but obviously either A) knew it was a riff based on Donovan's tune or B) was informed of the same at some time. Not sure if that's true, just somehting I've heard repeatedly from independent sources... none of whom have any particular credibility.

FWIW, The Gourds recorded a well circulated cover of Gin and Juice which is frequently (and erroneously) credited to Phish and/or Ween. Not sure if Hayseed Dixie also recorded a version, but it may be The Gourds...
