Oh, and one more deserving of mention is Mountain Jam by the Allman Brothers Band, granted their version has no lyrics. If you ever heard the original, 'There is a Mountain' by Donovan, you'll laugh your ass off when comparing it to the behemoth jam the Allman's turned it into.

This reminds me of another worst nominee: "Freebird/Baby I Love Your Way Medley" by Will to Power (the Big Mountain version is a weak cover too).

Cake - I Will Survive
Jeff Buckley - Hallelujah (Leonard Cohen)
Johnny Cash - a ton of his covers, but I particularly like One (U2) and Further On Up the Road (Springsteen)

worst:

Faith - Limp Bizkit
All valid choices. If a person doesn't get chills from Buckley's rendition of Hallelujah, then they simply don't have a pulse. My favorite Cash covers are NIN's Hurt and Soundgarden's Rusty Cage.

I don't claim to know alot about covers, but here are a few that I know of and enjoy:

The Ataris "The Boys Of Summer"
Wyclef Jean "No Woman No Cry"
Foo Fighters "Darling Nikki"
Joe Strummer & The Mescaleros "Redemption Song"
Like someone already mentioned, anything Johnny Cash has covered is good, but in particular "Hurt" and "Personal Jesus"
Gnarls Barkely "Gone Daddy Gone"
Pearl Jam "Teenage Wasteland"
