I believe they must. I think the guy had his tattoo done when in the "excited" stage. The average penis basically doubles in size from the flaccid state to the erect. So basically, when not aroused his tatoo probably plays out like this:

And of course, the 2" from 2002"

So, in the normal stage, without any excitement, his tatoo actually reads
