That's a commercial/pay option, but it's 100% effective since it's challenge-based.
I don't think anyone needs to resort to something like that though, unless perhaps it's a work-related email in a setting where you just can't afford to spend time sorting through crap.
I use K9 (linked in Mili's post above) on Mozilla Tbird, and these days I only have to correct it once out of every 5k emails or so -- and usually the correction is it flagged a good piece of mail as bad. So basically nothing unwelcome gets through.
Also, grab a secondary email (gmail, hotmail, whatever) and use only it for online forms when you purchase something, etc.
