Doesn't bother me a lick. Although it IS a natural process involving the excretion of bodily fluids, I don't put in the same category as urination, ejaculation, etc. Just doesn't seem like a big deal.

I think that as long as the kid is aged one year or less (maybe 2) it is fine to breast feed in public. For those of us from families who really do the extended breast feeding, I think that any breast feeding of older kids (ages 2 or 3 to 10) should only take place at home.
