Fairly obvious even at this early stage that the helicopter messed up big time and the pilot is responsible for the crash. It's not the airline's job to stay clear of the helicopter - he's got an obvious flight path he is required to stick to to land the plane. The helicopter literally can change direction at a moment's notice and would still get to wherever it is going.

In the end pilot error will always happen so there needs to be safeguards in the system. I read that Reagan usually has one controller for planes and another to monitor the helicopters, but at certain times of the day it's ok to have one person do both. Well that will never happen again.

The Lexington KY crash happened due to pilot error but the air controller wasn't watching the plane as he was dealing with "administrative tasks." From wikipedia:

At the time of the accident, the single controller in the tower was performing both tower and radar duties. On August 30, 2006, the FAA announced that Blue Grass Airport, as well as other airports with similar traffic levels, would be staffed with two controllers in the tower at all hours, effective immediately.

Yeah Thump I have seen all the YouTube animations of the flight crashes. Sometimes multiple times. The “worst” are the Alaska Airlines (faulty maintenance) and the Air France from South America (co-pilot panicked and basically crashed a well functioning Airbus from 40K feet)
